looks te me as Odontomyia ornata, that is it looks similar as O. ornata as we have in The Netherlands. I'm not sure if you have similar species in Slovakia, but I do not think so. Did you collect it along or in some water body or in a treehole?

I collected it in a small pool in flooded quarry. By Fauna Europea Odontomyia ornata lives in Slovakia also.
